After you register online for camp you will receive these required forms within your confirmation email. In order to participate in camp these forms need to be filled out, and brought to the first day of camp.

Bylaw 13.12.1.3 - A member institution's sports camp or clinic shall be open to any and all entrants (limited only by number, age, grade level and/or gender).
