Required Forms
Dear Parents / Campers,
After you register online for camp you will receive these required forms within your confirmation email. In order to participate in camp these forms need to be filled out, and brought to the first day of camp.
*Note: if you registered online and can't place the confirmation email attached forms within your email, please click the link below. If you mailed your payment in check/cash, please print the forms below and bring to camp.*

Bylaw 13.12.1.3 - A member institution's sports camp or clinic shall be open to any and all entrants (limited only by number, age, grade level and/or gender).
